 STAFF CHANGES JULY 2020 ANNOUNCEMENT

After being a GP Partner at Moordown Medical Centre for over eight years, Dr Chris Elfes has formally announced his decision that sadly he will be leaving us in July 2020. Dr Elfes has given excellent service and dedication to his patients and work colleagues alike. He will be greatly missed.

 

At the same time, we are delighted to announce that Dr Helen Wright will take over his role as a GP Partner. 

We are also currently advertising for additional clinicians to ensure we maintain our current capacity to offer a good service.

 

We would like to take this opportunity to thank Dr Elfes for all his hard work and support over the years and wish him good luck in the future. He is continuing to work in his other roles across medical assessment and education, but also intends to spend some time working abroad.

 

 

 

 

 

 

 

 

How To Register As a Patient 

We are pleased to accept registration requests from patients who are living within the area indicated on the map below.  If you are not sure if your address is included in our catchment area, please phone Reception on 01202 974700 to check you address is in our area before coming to the practice.

If you would like to register as a patient and live within our practice area; please bring your medical card, photo identification (such as driving licence or passport) and proof of address along to the surgery where you will be asked to complete a New Patient Questionnaire.  All patients at this practice are allocated a named GP (this is your registered GP)  You are still able/welcome to book appointments with the GP of your choice but your registered GP is responsible for your overall care.  Should you wish to know who this is please ask the receptionist.

Please obtain your NHS number from your previous GP surgery if you have one as this helps with the registration process.

If you change your name, address or telephone number please let us know by phoning Reception, writing to us or changing your details on SystmOnline.

If move outside the practice area, you will usually need to find a new GP practice.  You can find information on www.nhs.uk or your local library.
