HEALTH COACH AT THE PRACTICE
Managing life with a health condition isn't always easy. We know that understanding and keeping track of medications, treatments, tests and appointments with healthcare professionals are important, but also that a health condition affects our emotions, how we feel about ourselves, our social life, hobbies and day to day routines at home. Feeling confident managing all of that can be a challenge.
At Moordown Medical Centre, we have a health coach as part of our team. Clinical healthcare professionals like doctors and nurses can be really important to help us manage the medical aspects of our health, and health coaches can help us manage the emotional and practical impact in our day to day life.
The health coach is there to support people with health conditions to feel more confident, and have lots of experience talking about a whole range of subjects:
Our health coach, is based at The Moordown Medical Centre on Wednesdays each week, and is part of our team here to support you. Lots of patients and their carers say how hard it is when healthcare professionals only have ten minutes, but health coaches have up to 45 mins for each session to listen to you and support you.

Your GP practice nurse or any member of the reception team can refer you to see the Health Coach, so if you are interested in this, please mention it to one of the team at Moordown Medical Centre.
If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required. There is further information about countries and vaccinations required on the links below
We have travel packs available on reception to fill in, it is extremely important to complete this as early as possible - at least eight weeks before you travel. This is because an appointment may be required with the practice nurse to receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. The nurse will have a look through your travel pack and assess whether you need to be seen for any vaccinations. She will then contact reception and ask them to book an appointment for you. Your appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
As of the 1st of April 2017 the Dorset CCG have reviewed the provision of vaccinations that are available for travel and as a result we are no longer able to offer Hepatitis B in any form (including A/B combined). Any person needing this vaccination will be directed to a private travel clinic.
Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:
The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.
Important information regarding Firearms Licensing
Some of the GPs at the practice conscientiously object to gun ownership, and will therefore not be able to complete any paperwork in this regard. If you are requiring a renewal of a firearms license or applying for a new one please be aware that this applies to Dr Evans, Dr Turney & Dr Pollard.
The Practice shares your diabetes related data with the Diabetic Eye Screening Programme operated by Health Intelligence (commissioned by NHS England). This supports your invitation for eye screening (where you are eligible and referred by the Practice) and ongoing care by the screening programme. This data may be shared with any Hospital Eye Services you are under the care of to support further treatment and with other healthcare professionals involved in your care, for example your Diabetologist.
