How To Register As a Patient
We are pleased to accept registration requests from patients who are living within the area indicated on the map below. We release New Patient Registration packs every Tuesday morning on a first come first served basis - these can be collected by a third party should you be unavailable to attend yourself. We do not post or email out these registration packs, they must be collected from the surgery.
Moordown Medical Centre has around 9,000 patients registered with us. We register a small number of new patients each week which is usually dependent on the number of patients leaving the Practice in order to maintain stable numbers and ensure the good quality service we aim to provide for our patients.
If you would like to register as a patient and live within our practice area; please bring your medical card, photo identification (such as driving licence or passport) and proof of address along to the surgery where you will be asked to complete a New Patient Questionnaire. All patients at this practice are allocated a named GP (this is your registered GP) You are still able/welcome to book appointments with the GP of your choice but your registered GP is responsible for your overall care. Should you wish to know who this is please ask the receptionist.
If you do not have your Medical Card, you will still be able to register.
